Functional Behavioural Assessment & Positive Behaviour Support Plan

ASSESSMENTS

1. Include an Introduction and Rationale for a behavioural support plan. Make sure that it is professionally written and formatted.  This document is a reflection of you as a professional. Include the following:

1. 1. Demographics
   2. Purpose of assessment / report
   3. Brief description of referral target behaviours, diagnoses, and other relevant information.
   4. Location (e.g., school, home, etc.) and environment.
   5. Avoid talking about procedures here
   6. Describe informed consent received (who signed, relationship to client, from where, date of signed consent) and information letter provided to consent source and agency.

Indirect Assessments

1. Functional Assessment Interview (FAI):  Provide a brief summary of sections A-J (not K) of the FAI (without overlapping with any other sections in the assignment).  Each section must have a heading followed by a brief summary.  Try to stick to 1 or 2 target behaviours maximum.  Do not include a written copy of the FAI in the Appendix.

• Note: Provide clear, distinct, and descriptive operational definitions for the target behaviour(s) before you include subsequent information based on the FAI.

1. Questions About Behaviour Function (QABF):  Summarize the results using the following table for each of the target behaviours and comment on the results:

Direct Assessments

1. Baseline ABC & FIDD Data:  Provide 5-7 days of ABC data. Summarize the results of the ABC data and include observation period dates and locations. Include the raw ABC data in the Appendix and not in the report.  You may summarize other data collection utilized as part of the assessment here (i.e., scatter plot data, inter-observer agreement data, quality of life measures, social validity measures, etc.).

• List possible early warning signs of the target behaviour(s)

• List MOST likely setting events and antecedents for the target behaviour(s)

• List LEAST likely setting events and antecedents for the target behaviour(s)

• List probable consequences maintaining the target behaviour(s)

FIDD data should be based on your baseline ABC data.

6. Graph of Baseline Data: Create a graph with a behavioural title, X axis (i.e., frequency, percentage, etc. of the target behaviour) & Y axis (i.e., days). Plot your baseline data and ensure that you indicate a baseline dash mark (condition line) at the end of your data.  Ensure that you have a legend specifying which symbol is for which behaviour.  See your text book for examples.  Refer to your Cooper ABA Book for more information.

An excel graph is preferable.

7. Hypothesis / Summary Statement Narrative: Do one for each target behaviour.

Sample Hypothesis Statement:

When ___________ (client’s name) is ________________ (antecedent) in  ____________ (setting / situation), he/she _____________ (problem behaviour) for ____________ (function).  This pattern is more likely when _________ (client’s name)

has ______________ (setting event).

Note: Ensure you comment about what the function is for each target behaviour.  You may include more than one hypothesis statement depending on the amount of target behaviours you have with similar or different functions.

8. Summary Statement / Competing Behaviour Pathways Diagram

Complete the diagram with the information gathered in steps 1-7.  Provide a diagram for each target behaviour.

9. Positive Behavior Support Plan

Strategies that Make Problem Behaviours Irrelevant, Ineffective, and Inefficient.  You may include headings for each type of strategy category with bulleted or numbered recommendations.  Justify your choice of strategies by citing 2 relevant journal articles from JABA (APA style).
