She also stated that her appetite had decreased significantly since starting the medication and she has lost 15 pounds in total. The patient said she felt extremely weak at times during her day-to-day activities such as getting dressed or walking up stairs; however, she did not mention any other symptoms besides fatigue and decreased appetite during this visit.

Upon examinaton , the patient’s vital signs were stable except for blood pressure which was elevated (+3/0 mmHg), Glasgow Coma Score (GCS) score of 15 indicating mild confusion following sedation due to previous hypotension (blood pressure <90 mmHg systolic/diastolic), temperature 99 degrees Fahrenheit (+1 degree Celsius); pulse rate 70 beats per minute (+2 beats per minute); respiratory rate 22 breaths per minute (-2 breaths per minute)

There were no abnormalities noted on physical examination with regard to hearing tests being performed without any abnormal findings noted specifically regarding hearing loss but rather just outlying normal findings within this particular exam
