C. Stewart is a 47-year-old female with a diagnosis of depression. She has a history of multiple suicide attempts and non-compliance with treatment. Her medication treatment has included Latuda, Zoloft, Wellbutrin, Paxil, Bupropion HCL. She refuses staff interventions and requires redirections and prompts, ques, and reminders. The resident refused the prescribed medications because they made her gain weight. One problem that did not resolve was the drug kept her gaining weight. Residents have the right to refuse medication, and the nurse and other team members have to honor the patients’ decisions. One effective nursing intervention was using the reward system. The resident would be rewarded for medication compliance, and this works because the resident enjoyed smoking and the temporary release that came from it. This action kept the client safe from harming herself with further suicide attempts. The resident voluntarily came for medication administration to receive the reward. She has had no other suicide attempts or ideations. She has displayed a labile mood with no behavioral outbursts or events.

Uju, a 25-year-old married woman, recently gave birth to a stillborn child. A month before her due date, she noticed that the baby’s movements were sluggish and decided to see her doctor. The baby died during delivery. After resigning from her job, Uju dedicated herself to crying every single day. Because she was constantly thinking about what was wrong and whether she was to blame, she found it difficult to concentrate. Gradually, she neglected self-care, slept excessively, and had constant anxiety. Her appetite decreased and she was perpetually exhausted. According to her, nothing is going right in her life, and she is constantly depressed. Her suicidal thoughts occur occasionally when she loses all desire to live. Considering her symptoms and the events that precipitated her mood disorder, Uju’s doctor diagnosed her with major depressive disorder. All of the medications prescribed are SSRIs (Selective Serotonin Reuptake Inhibitors), including fluoxetine (Prozac), sertraline (Zoloft), and citalopram (Celexa). Since she needed a baby and didn’t take her mind off of her situation, these medications did not help. Instead, she kept looking at all the baby things she had already set up.

The healthcare team, her husband, friends, and spiritual leaders regularly talked to her and prayed with her as part of her psychotherapy. The nursing intervention that helped was weekly counseling sessions with other mothers who had similar problems. In addition, she had sessions with her gynecologist. By providing supportive resources, these nursing interventions helped alleviate her feelings of hopelessness and isolation. Her mother’s support and psychotherapy made her feel better, so I believe she was kept safe
